Submitting an Access Request Form via the DHTS User Portal (Service Now)

Once these forms have been completed, submit an Access Request via DHTS User Portal (ServiceNow).

 

  1. Navigate to  Get IT > Analytics & Reporting > Request for Accounts/Access & Training for Reports and Data
  2. For the Service: option, select ‘SQL Access to DSR/Clarity or Caboodle’
  3. Complete all sections of the form
  4. If your access will be via PACE, include that in the Detailed Business Objective section
  5. Click the Add Attachment button or the paperclip to attach your Agreement Form and Use Case Form

A service account is an account used for programmatic access to data such as from a web site, application, or automated data extract process. Typically you don’t want those processes tied to a developers account since the account would be dropped when the developer leaves. This is particularly true if it’s also used as a persistent data store of user managed data.

Prerequisite:

  • Forms must be signed by department manager.
  • Each developer who has access to the Service Account needs to have their own individual account (see above).

NOTE: Service Accounts are not allowed to be shared between staff as a way to access data outside of what is needed to support the web site, application, or automated data extract.

 

Submitting an Access Request Form via the DHTS User Portal (Service Now)

Once these forms have been completed, submit an Access Request via DHTS User Portal (ServiceNow).

 

  1. Navigate to  Get IT > Analytics & Reporting > Request for Accounts/Access & Training for Reports and Data
  2. For the Service: option, select ‘SQL Access to DSR/Clarity or Caboodle’
  3. Complete all sections of the form
  4. If your access will be via PACE, include that in the Detailed Business Objective section
  5. Click the Add Attachment button or the paperclip to attach your Agreement Form and Use Case Form

 

*** Any of these forms must be completed and then attached to the Access Request form in ServiceNow.
